Allergysmarts recognizes your right to have your personal information safeguarded and we are committed to protecting your privacy. Allergysmarts complies with all provincial and federal laws in relation to the protection of personal health information, as per the “Health Information Act†of Alberta and the “Personal Information Protection and Electronic Documents†Act of the Government of Canada. We provide our U.S. patients with the same full protection in the collection, use and storage of their personal health information. All of our employees are advised and trained on our corporate privacy policies and of the importance of confidentiality with respect to customer information and in the use, maintenance and dissemination of such information. We sign Confidentiality Agreements with all of our representatives/employees, nutritionists and laboratory technicians. As part of our Enrollment form and agreement with our customers, we request our customer's written authorization and consent to collect, use and share such information for the limited purpose of performing a food intolerance test and consulting on the customer's behalf. Allergysmarts computer system uses the latest in technology to achieve and maintain the highest professional standards for confidentiality and integrity. Internal access to all systems is secured by a user ID and password logon. The server operating systems, including email are secured with up-to-date Antivirus systems. All of the information, and we have multi-layered access control to prevent unauthorized users and devices from accessing data or machines in our network. All of our data is backed up daily and moved to offsite secured storage. We do not sell or otherwise provide customer information to any other business entity or marketing entity. All blood samples provided to us by customers are destroyed 30 days after food intolerance testing has been performed. The Information We CollectIn order to process your order and perform a Food Intolerance Kit, it is necessary for us to ask you to provide us with some personal information. This information includes your name, address, telephone number, email address, date of birth, existing medical conditions and any known allergies. Additionally, since you are using a credit card to place an order for your Food Intolerance Kit from us, it is necessary you provide us with your credit card information. Credit card information is used for payment purposes only. Lastly, the collection of dry blood samples is used ONLY for the purposes of performing a food intolerance test and all blood samples provided to us by customers are destroyed 30 days after we have performed the food intolerance testing. The Way We Use Your Information:Information that you provide to us when placing an order for your Food Intolerance Kit is used only to process your order. It is not shared with third parties except to the extent specifically required to complete the order. For example, it is necessary for us to share your name and address with our shipping department and your name and address must be disclosed to the postal service in order to ship the Food Intolerance Kit to you. In addition, we require your email address so that the laboratory may send you your test results. Email addresses are not used for any other purposes and are not shared with third parties. The only exception to this is if you send us question/comment by email then we will use your email address to respond, or if you request that we send you one of our newsletters in which case we will then use your email address to send you such newsletters. Your blood samples are sent directly to our laboratory and are handled only by our laboratory technicians. Once all of the tests have been completed the results of your test are provided only to one of our nutritionists and yourself. The nutritionist requires the information only so that s/he can provide you with a personalized consultation on the results of your test. Note that customer service staff cannot access your test: any questions regarding your results should be discussed with the nutritionist during your consult. We will not release any test results or customer information to any other person, other than to the customer who ordered the testing kit, or to the customer's authorized guardian, where such guardian must be identified at the start of the ordering process Return PolicyAll sales are final! Once a Food Intolerance Kit has been mailed to you we shall not accept the return for use or re-use any portion of the kit, due to safety and to ensure that no tampering has occurred with the kits which may then affect test results.
